Reserve Material Request Form

  • Please use a separate form for each course. Submit this form electronically for materials held in the Jennie Huizenga Memorial Library or attach a printed form to instructor-provided materials.

    Reserves will be processed within one week of submission date. A backlog at the beginning of the fall and spring semester can delay processing times.

    If you are dropping off personal copies to place on reserve, please give to Sarah Hoeksema, Circulation & Access Librarian.

    If you would like the library to purchase materials, please fill out the Library Material Purchase Request Form, making sure to indicate that these materials should be placed on reserve for your class.

    Please provide the following information:

  • Include Title, Author/Editor, Edition, Format, # of copies, ISBN, etc.
  • MM slash DD slash YYYY
  • MM slash DD slash YYYY
  • All reserve materials must meet the standards of educational fair use as outlined in Section 107 of copyright law. Materials obtained through interlibrary loan may not be placed on reserve.

    While the library strives to keep all materials secure, be aware that instructor-provided copies may be damaged or lost during the course of the semester due to theft and/or student use. The Jennie Huizenga Memorial Library cannot compensate you for the value of your personal materials nor replace them.
